CARES of NY, Inc. (CARES) collaborates with and supports our communities towards the creation of systems that end and prevent homelessness. We envision that all people have access to safe, affordable housing and the social supports to remain housed.

CARES offices are in Albany, NY, however our customers and partner agencies are located throughout 37 counties in NYS. That is approximately 65% of the State. We have been growing since 1990, with most of our new hires made recently.

POSITION TITLE: Case Manager

DEPARTMENT: Housing

REPORTS TO: Supervisor of Case Management

SUMMARY

The Housing Case Manager is responsible for supporting clients in maintaining stable housing. The position provides support to community members who have experienced episodes of homelessness and works to ensure clients have access to high quality support services. Activities conducted by this position fall within the following three categories: 1) Care Coordination and support service referrals 2) Individualized service plan development and review and 3) Administrative program oversight and regulation compliance.

ESSENTIAL DUTIES & RESPONSIBILITIES

  1. Determines if referrals meet eligibility guidelines for the housing program
  2. Provide services directly to clients or determines with supervisor if it is more appropriate to refer to a partner (e.g., substance abuse and/or mental health counseling).
  3. Responsible for scheduling and running monthly meetings with medical providers to coordinate and track all mutual client services.
  4. Tracks client viral loads bi-annually.
  5. Ensure clients receive medical care as well as get them reengaged in medical care if needed.
  6. Ensure clients find and maintain stable housing with their housing voucher
  7. Responsible for ensuring up to date information within the Homeless Management Information System (HMIS).

EDUCATION

Bachelor's degree required.

EXPERIENCE

At least one year of direct case management experience.

Additional Experience: Experience working with homeless populations.

  9. Experience with MS Word, Outlook is required; Excel, OneDrive, SharePoint preferred
  10. Must possess excellent communication skills, both orally and in written form.
  11. Must have a clean, valid NYS license

LOCATION

Albany, NY

The individual hired for this role is expected to be onsite four days per week.
